We ONLY process your order with option 1 or option 2

1. The credit card holder fills out the credit card authorization form and sign his/her name on the form Credit Card Authorization Form, also we need the copy of your credit card statement, please feel free to cross out any informaiton you feel is personal. We are only interested in seen your credit card number on the same document as your name and billing address.

2. The credit card holder fills out the credit card authorization form and sign his/her name on the form Credit Card Authorization Form, also we need a copy of the back side of the credit card

You can send the form and copy either by scan to the computer and email with attachment to us at toycup2@yahoo.com OR you can fax them to 1-510-687-9951. Your order will be automatically canceled after 14 days from the date you ordered if we didn't get the documents we need!
